REGISTRATION AND LIABILITY RELEASE FORM

 I  FirstName LastName   SUBSCRIBE TO AND ACCEPT THE FOLLOWING:

 Rei No Dojo Inc. and its teaching and administrative staff shall not be liable for any damages arising from any personal injuries sustained by a guest or a client on or about the premises of Rei No Dojo at 562 Ontario St..  A guest or a client, in attending Rei No Dojo and using the facilities and equipment, does so at his/her own risk.  I freely acknowledge that I am fully aware that Sylvanus Klotz and his associates are not medical doctors and that any suggestions given should not be taken as medical advice.  If I am seeking a MEDICAL solution I will consult a doctor or specialist.

 A client or a member assumes full responsibility for any injuries or damages which may occur to him/her using said facilities and he/she does hereby fully and forever release and discharge Rei No Dojo and its teaching and administrative staff from any and all claims, demands, damages, rights of action, or causes of actions, present or future, whether the same be known or unknown, anticipated or unanticipated, resulting from or arising out of a member’s or a client’s use or intended use of said facilities and equipment.

 I warrant, represent and agree that I am in good physical condition and have no disability, impairment, or ailment preventing me from engaging in active or passive exercise or any complimentary therapy or service offered at Rei No Dojo that will be detrimental or inimical to health, safety, comfort, or physical condition if I do so engage or participate.  Rei No Dojo and its teaching and administrative staff shall not be liable for the loss, theft or, damage to the personal property of a guest or a member.

 CLIENT RESPONSIBILITY: Due to the nature and quality of the work being done at Rei No Dojo, the schedule of the teachers/facilitators is rigid and must be adhered to.  There are a limited number of sessions available each day.  Because of this clients must keep to their appointment times by arriving 10 minutes early for their session.  Late clients will have their sessions shortened in order to keep to the schedule, out of respect for the instructors and clients.  Clients who do not show up for their session or who have not given sufficient cancellation notice (i.e. see cancellation policy) will be charged for their session.  A client with an outstanding fee for a missed session or an unpaid session will not be rebooked until all accounts have been settled.

I FirstName LastName have read and agree to keep and adhere to all the policies and procedures including cancellation policies now in force or prescribed by Rei No Dojo for the use of its facilities and equipment.  It is my responsibility to keep up to date with any changes in policies and procedures.
